Prior to Your Visit
Prepare for a seamless appointment experience by creating an online patient portal account and filling out required forms in advance. If you prefer to complete forms in person, please arrive 15–20 minutes before your scheduled appointment time.
1. Create an Account in the Online Patient Portal
To create an account, visit the patient portal welcome page and click “Create an Account.” This will take you to the registration page, where you have the option to register as a patient or their proxy. Enter the patient’s basic information to verify their identity and set up the account.
Complete the Following Forms in the Patient Portal
- HIPAA Privacy
- Consent to Treat
- Consent to Bill

Day of Your Visit
Bring the Following to Your Appointment
- Valid photo identification
- A list of your medications, including names and dosages
- Insurance card(s) and co-pay
Check in to Your Appointment
Check in online via the patient portal and at the front desk when you arrive.

Convenient free parking options are available near our clinics. Please allow extra time to find parking and check in before your appointment.

Cancel an Appointment
If you need to cancel an appointment, 24-hour notice is required. Please call 509-505-7481 to cancel or reschedule. This allows us to better meet patient needs.
